WHAT IS WRAPAROUND?

"Wraparound" is a family program which helps families put together a family care plan. When your family is facing difficulties, "Wraparound" can help. Bridges for Children knows that every family is unique, so every family will have a different plan. That's the best thing about "Wraparound"; it's all about your family's strengths and needs.

 
WRAPAROUND IS NOT...

... just another type of service or program. It is not another agency for financial support. Wraparound will help make your family stronger and will give you hope for tomorrow.

 
WHAT DOES IT LOOK LIKE?

Wraparound looks like a blanket of caring people "wrapped around" your family. Those people believe in you and want to help your family face your difficulties.
Your Family will work closely with a Wraparound Coordinator. Your family and the Resource Coordinator will decide who will be on your Family Team.

Together, you will make a family care plan.

 
WHAT IS A FAMILY TEAM?

Your Family Team will be made up of people who know you well. This group of people will work with you to make a family care plan. The Family Team makes a commitment to support you and your family - no matter what.

 
WHO IS ON THE FAMILY TEAM?
  • YOUR FAMILY is always at the center of the Wraparound process. Every idea shared and every part of the care plan is there to help your family with the things that you think are important.
  • THE COORDINATOR is a person who helps you. The Coordinator will partner with your family as the care plan is put into practice. The Coordinator is the person that will provide the most contact with your family. They will also lead the Family Team meetings.
  • OTHER TEAM MEMBERS will include people who know you and are of help to you. Team members may include neighbors, family, teachers, agency workers, etc. (For example: you may want someone from your child's school to be on the family team.)

 
WHO MAKES THE FINAL DECISION?

Wraparound is all about your family, so your family has the final say in every part of the process. Each Family Team member and each part of the care plan must have your approval.

 
HOW DOES IT WORK?

Step #1 - Getting Involved

The Coordinator will discuss your family's strengths and needs with you. You will also begin to decide who will be on your Family Team.
The Coordinator will help contact these people and arrange a Family Team meeting.

Step #2 - The Team Meeting

Your team will meet to develop your family care plan. You and your team will talk about your strengths past successes and local supports. Then the team will talk about your concerns; and together you will form a care plan to meet your needs. That means your plan will try to make use of all the things that make your family special, strong and capable.

Step #3 - Carrying Out the Plan

Once the plan has been written and signed by all the team members, the team will begin working to carry it out. The Coordinator acts like the coach of the team to make sure every team member is doing their part of the plan.

Step #4 - Progress Review

The team will get together regularly to see what has been working and what still needs to happen. If new concerns come up new goals will be made. This process will contune until your family believes your golas have been reached.
